Your Medical Information

At Sturdy Health, it’s important that our patients have access to their medical records and information, both easily and securely. Your records are available through the Health Information Management department located on the ground floor of Sturdy Memorial Hospital. Additionally, you have access to your information through a secure online patient portal.

Medical Records

The Health Information Management department is available to provide information from your medical record at Sturdy Memorial Hospital only. Copies of birth certificates may be obtained from Attleboro City Hall. Download the Authorization to Release Health Information [PDF] document, should you require it.

Accessing the Patient Portal

Sturdy Health’s patient portal MySturdyHealth, gives you secure online access to your medical information. The main features of the portal allow you to electronically:

  • Reference, manage, and print medical records and information related to medications, allergies, appointments, procedures, emergency department visits, lab and other test results, and more
  • Share medical information with clinicians and other people you authorize
  • Receive reminders for appointments and for preventive and follow-up care
  • Access discharge instructions and educational resources personalized to individual medical needs

The portal is easy to navigate with interactive menus and tools that make retrieving personal health information relatively simple. The portal’s safe and secure data access and transmission adhere to Health Insurance Portability and Accountability Act (HIPAA) privacy rules and regulations.
