CHNA Reporting
A Community Health Needs Assessment (CHNA) is a systematic process involving the community to identify and analyze community health needs. The process provides a way for health systems like Sturdy Health to partner with the communities they serve to prioritize health needs, and to plan and act upon unmet community health needs.
Completing a CHNA every three years allows us to keep a pulse on the public health concerns facing our friends, family and neighbors who live within our official service area and develop strategies to address them.
Sturdy Health’s Board of Managers approved the 2022 Community Health Needs Assessment and Implementation Plan for FY2023-FY2025 on October 24, 2022
Key Health Issues Identified In 2022
Our 2022 CHNA identified 5 Key Community Health Issues in our Sturdy Health service area, including the following:

- Affordable Housing and Income
- Mental/Behavioral Health and Substance Abuse
- Mortality and Chronic Disease Management
- Obesity and Weight Management
Our Improvement Plan Priorities
Based on feedback from community partners, including health care providers, public health experts, health and human service agencies, and other community representatives, we committed focus our community health improvement efforts on the four health- and wellness-specific priorities over the next three-year cycle.
